When is the last day to enroll for a first of the month effective date?

The 25th of the month.

One rate for all ages?

Yes, currently one set of rates for all ages.



What are the minimum and maximum ages?

18-64.



How many children can be covered under the family plan?

Unlimited number of legal dependents/children can be covered.



Is this product available in all 50 States including CA?

Yes, these plans are available in all 50 states including CA.



Do you need a S.S. # number to enroll?

No, you do not need a social security number, you can use a TIN number instead.



Do members need to join an LP or download a browser?

NO, there are no LPs or browsers required.



Are doctor visits shared?

NO, doctor visits are per member on the plan. If there are 10 doctor visits, each member on the family plan will receive 10 visits.



When do members receive their cards?

Members receive their temp cards immediately upon enrollment.  They will receive their physical cards 15 business days after their effective date.



Methods of payment?

Credit card and ACH are accepted.



Can I upgrade or downgrade?

Yes, you can downgrade anytime but you can only upgrade during the open enrollment period November 1st through December 31st.

Ct. 747 (1969) affirm that litigants may be assisted by unlicensed laymen during judicial proceedings. Competent non-lawyers from groups can aid other members of the group in court without facing charges of "Unauthorized practice of law."
